Scottsdale's Medicare population skews older and considerably wealthier than the valley average, and that changes which parts of Medicare actually matter. Here the conversation is rarely about affording Part B. It is about IRMAA, provider access and whether a network plan is worth the trade.
IRMAA is the Scottsdale question
If your 2024 income was above $109,000 filing single or $218,000 jointly, you pay an income-related surcharge on top of the standard Part B premium — and an additional amount on Part D. The top tier reaches $689.90 a month for Part B.
Two things people miss. First, it is based on your tax return from two years ago, so a one-off event — selling a business or a property, a large Roth conversion — can raise your premium for a single year well after the money is gone. Second, if your income dropped because of a life-changing event such as retirement or the death of a spouse, you can appeal using Form SSA-44. Advantage or Medigap
Scottsdale residents often have both the means and the inclination to prioritise access over premium. That tends to favour Medigap: any Medicare-accepting provider nationwide, no networks, no referrals, and no annual out-of-pocket surprises beyond the Part B deductible.
The counter-argument is real though. Maricopa County's Advantage market is competitive, with $0 premiums and extras that Medigap does not include. Which is right depends on how much you travel and how attached you are to specific specialists.
The timing that catches people
If you retire later than 65 with employer coverage, the Part B Special Enrolment Period and the six-month Medigap window both key off when Part B starts — not off your birthday. Getting that sequence wrong is expensive and hard to undo.

If your income fell because of a life-changing event — retirement, the death of a spouse, divorce, loss of income-producing property — you can appeal using Form SSA-44 and ask Social Security to use current income instead of your 2024 return. If your income was simply higher two years ago with no qualifying event, the surcharge stands until your later return works through.
Is Medigap worth the higher premium in Scottsdale?
